ADHA and Colgate are pleased to announce the Colgate/ADHA Community Outreach Award. This component award program recognizes ADHA components and individuals submitting on behalf of components that have implemented significant community outreach programs, focusing on preventive oral health care.

One recipient will be selected. Recipient will receive the following:

• Commemorative award
• Recognition in an ADHA publication
• Complimentary registration for Annual Session
• Recognition at Annual Session
• Two days' per diem at Annual Session
• Air travel to Annual Session
• Two nights' lodging at Annual Session
• $2,000 donation to component
