DUE WEDNESDAY, MAY 9, 2012:

- ADHA to forward meeting room assignments/confirmed room
      setup/speaker manual to all Speakers

DUE WEDNESDAY, MAY 16, 2012:

- FORM D – Handout/Internet Waiver

- ADHA to forward hotel confirmations to all Speakers, if applicable

 

DUE WEDNESDAY, MAY 30, 2012:

- FORM E – Shipping Information

AUGUST 2012

WEEK OF AUGUST 20, 2012:

- ADHA to forward Speakers post-meeting evaluation tabulations

 

Room Setups:
ADHA will accommodate the speaker’s requirements as best as possible. Due to the limited capacity of room sizes, many courses will be set up in theater style. Be sure to indicate your room preference/any special requirements on the room setup/AV request form in your speaker packet.

ADHA will provide the following AV setup in each meeting room:

  • LCD Projector/Screen
  • Podium w/microphone
  • Wireless Lavaliere Microphone
  • Flipchart w/Markers

Be sure to indicate any special AV requirements for review/consideration on the room setup/AV request form in your speaker packet.

In early May, ADHA will provide you with the confirmed room name/room diagram (setup) for your course.

 

Handouts:
Due to ADHA on or before May 16, 2012.

A handout is required for all courses and must be submitted on or before May 16.

If you do not wish to provide a handout to be posted online, a one page course outline must be provided to be posted online by the May 16 due date.  ADHA requires all courses to have either a handout or a one page course summary available for distribution/posting on the website. All handouts must be submitted to ADHA for approval.  Handouts are not to exceed 10 double-sided pages (6 slides per page, if a PowerPoint presentation).

ADHA will post the one page outline or handout online for all meeting attendees to download.

ADHA will copy the handout for distribution to all participants registered for your CE course.

 

PowerPoint Presentations:
ADHA recommends utilizing the first and last slide template provided to you for your presentation. These slides will be sent to you in April 2012 as they are specific to your CE course.

Registration/Housing:
All speakers receive complimentary main base registration.   If a speaker would like to attend any other CE course, they can do so at the prevailing rate.

ADHA Member Speakers – please loggin with your member number to complete your registration. Your main base fee will appear at no-cost.

Non-ADHA/Dental Hygienist Speakers – ADHA will provide you with a loggin to complete your registration. Your main base fee will appear at no-cost.

Please refer to your individual speaker agreement to review the housing arrangements.
